Implementing the New 2019 ASCCP Risk-Based Management Consensus Guidelines for Abnormal Cervical Cancer Screening Tests
The long-awaited new ASCCP Management Guidelines have been published and an App is now offered. These guidelines lay out recommended follow-up for all three available cervical screening strategies: primary human papillomavirus (HPV) screening, co-testing with HPV testing and cervical cytology, and cervical cytology alone.
The new guidelines are described in three articles published in the Journal of Lower Genital Tract Disease in April 2020. Revised to reflect a substantial amount of additional data from the United States, the updated guidelines have several important advancements from the 2012 guidelines. Firstly, they have been streamlined for ease of use. The guidelines rely on individualized assessment of risk for precancer (CIN3+), taking into account past history, current results, and personal factors which allows for a more complete and precise estimation of risk. The recommendations aim to provide more appropriate intervention for high-risk individuals and less intervention for low-risk individuals. The use of risk-based “clinical action thresholds” allows the guidelines to be easily updated in the future with the emergence of new data and risk modifiers and technologies.
